Pharmaceuticals and Nutraceuticals

In the upcoming sections, we'll cover several nuances of pharmaceutical and nutraceutical options for treating psychiatric symptoms. Each lesson focuses specifically on a particular class or group of agents. Each lecture will detail mechanistic considerations, indications, special population et al. considerations, tolerability, option navigation, dosing, and course of treatment.

Objectives:

  1. Describe the mechanistic variations, indications, side effects, special population considerations, and dosing of pharmaceutical medication options.
  2. Synthesize a personalized plan of care considering treatment option nuances and special considerations.
  3. Apply nutraceutical options as a monotherapy or augmentation strategy based on the patient's experiences.
  4. Navigate through issues arising in the course of treatment using the TEAM acronym.

I have developed an acronym, "TEAM," for treatment considerations. When you follow up with a patient, what are some things to focus on? This acronym stands for:

  • Tolerability: An extension of the side effects discussed in the lecture with the inclusion of key pivots to help navigate side effects.
  • Efficacy: Considerations for determining the efficacy of treatment.
  • Adherence: Addressing general adherence issues as well as withdrawal and overdose.
  • Monitoring: Labs, diagnostics, and screening tool considerations.

I like this acronym as it reminds us that it takes a "TEAM" during treatment—at minimum, you and the patient, but this also includes family, friends, and the therapist.

Supplement Dispensaries

As you begin to utilize more herbals and botanicals in your practice, a dispensary can be helpful. Fullscript (https://www.fullscript.com/) is a dispensary where you can place nutraceutical orders for patients. Patients will receive an email where they can fill out their information and have the product sent to their home.

Once you sign up for Fullscript, you can provide up to a 35% discount for patients who order through your account. You also have the option of earning profit through Fullscript as well. You can set the discount lower and receive the difference as added profit. For example, if you set the discount to 15%, then your profit will be 20%.

Fullscript is integrated with some electronic health records (e.g., Charm), where you can order nutraceuticals directly in the patient encounter or from the EHR without having to log in separately and enter information into Fullscript.
